The Website is provided for general informational and educational purposes and is not a substitute for professional medical care. Viewing or using the Website does not establish a provider–patient relationship. 

    2. PATIENT CARE & OPERATIONAL POLICIES

    The clinical care, telehealth, prescription, laboratory, billing, and emergency policies of KaleidaCare Health & Wellness, including detailed telehealth consent and patient responsibilities, are set forth in the Patient Intake Packet and related consent forms. By using this Website or Services, you acknowledge that these operational and clinical policies are provided separately and that this Terms of Use document governs only your access to and use of the Website or Services, not your medical care.

    3. PRIVACY POLICY

    KaleidaCare is committed to protecting the privacy and security of your information. Use of the Website signifies your agreement to our Notice of Privacy Practices and Privacy Policy, which are available on this Website.

    4. USE RESTRICTIONS

You must review and comply with those materials separately.

    To the fullest extent permitted by law, KaleidaCare disclaims liability for any damages arising from your use of the Website or Services, including reliance on information provided through the Website or inability to access the Services. This applies regardless of the legal theory involved and even if we were advised of the possibility of damages.

    KaleidaCare reserves the right to investigate and take appropriate action against any user who violates these Terms, including suspending or terminating access to the Website or Services.

    Get a list of those with whom we share your information

    You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why. We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We will provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

    Choose someone to act for you

    Your Choices
