Viewing or using the Website does not establish a provider–patient relationship.
2. PATIENT CARE & OPERATIONAL POLICIES
The clinical care, telehealth, prescription, laboratory, billing, and emergency policies of KaleidaCare Health & Wellness, including detailed telehealth consent and patient responsibilities, are set forth in the Patient Intake Packet and related consent forms. By using this Website or Services, you acknowledge that these operational and clinical policies are provided separately and that this Terms of Use document governs only your access to and use of the Website or Services, not your medical care.
3. PRIVACY POLICY
KaleidaCare is committed to protecting the privacy and security of your information. Use of the Website signifies your agreement to our Notice of Privacy Practices and Privacy Policy, which are available on this Website.

We will provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.
